Record

Where the child has contributed their views, record in detail what they have said, using their own words wherever possible.

Include within the record a clear indication of the context in which the child communicated their views, who was present, where it took place, what you asked them, the duration of the discussion, etc.

Be balanced, fair and representative – i.e. don’t only include comments from the child that support your hypothesis and don’t minimise or explain away their comments unless you have clear evidence and defensible reasons for casting doubt on the views they express.

Record what communication methods have been employed and to what extent these have optimised the child’s opportunity to contribute their views.

Have you recorded a clear demonstration of the work done? i.e. logged the number of times you have seen the child, what you did with them on each occasion and how this contributed to the assessment and decisions.

Have you identified all sources of evidence and material (including research or things that others who know the child have told you) within your analysis and recording?
